Orderlies (patient transporters and support aides, in modern HR-speak) keep a hospital physically flowing: wheeling patients to imaging and surgery, delivering specimens to the lab, restocking supplies, shifting beds and equipment, and helping lift and reposition patients who can't move themselves. It's unglamorous, essential logistics with a human being at the center of most trips — an anxious pre-op patient wants a calm person pushing the gurney, not a beeping cart.
Hospitals have quietly become one of the most robot-dense workplaces in America. Autonomous mobile robots already haul meds, linens, meal trays, and lab specimens through corridors and elevators around the clock, and every large health system pursuing labor savings has a pilot or a fleet. UV-disinfection robots took over part of the cleaning workflow during the pandemic and never left. The pure-materials portion of the orderly job — which is a large fraction of the shift — is the exact use case these machines were built for, which puts our risk score at 72.
The resistant core is patient contact and patient handling. Transporting a person means monitoring their IV line, noticing they've gone pale, reassuring them, and safely transferring a 250-pound post-surgical patient from bed to gurney — tasks where robots assist (powered lifts, transfer devices) but don't replace human judgment or hands. Chronically short-staffed hospitals will also happily reassign surviving orderlies toward patient-facing work rather than lay them off outright. The realistic future: fewer orderlies per hospital, each doing almost entirely patient-touching work while the robot fleet handles everything that doesn't have a pulse.

This one is mid-disruption. Delivery robots are operating in hundreds of hospitals now, and materials-transport tasks will be substantially robotic at large systems by the late 2020s. Patient-transport and lifting work erodes far more slowly — expect roles to be redefined around patient contact rather than eliminated wholesale, with total headcount drifting down through 2030 as attrition does the quiet work layoffs would otherwise do.
Shrinking rather than disappearing. Delivery robots are absorbing the supply-running and specimen-courier portions of the role at large hospitals right now. But patient transport, lifting, and repositioning still need trained humans, and hospitals are chronically understaffed on the patient-care side. Expect fewer total positions, refocused almost entirely on tasks involving actual patients.
Robotic beds and automated transport concepts exist, but hospitals aren't handing an anxious, IV-tethered human to an autonomous gurney anytime soon. Patient transport requires monitoring the person's condition, managing lines and oxygen, and responding if something goes wrong en route. Robots take the cargo runs; humans keep the patient runs for the foreseeable future.
Climb toward the patient. A CNA or patient care technician certification typically takes weeks to months, often paid for by the hospital, and moves you into work automation can't touch. Sterile processing, EMT, and imaging assistant paths are also realistic jumps. The worst position is staying purely on materials transport, because that's the robots' home turf.
The parts that touch patients are among the safest jobs anywhere — demand is growing as the population ages, and robots remain terrible at physical care and comfort. The parts that move objects are among the most exposed, because hospital corridors are exactly the mapped, controlled environment robots handle well.
